Implementing the Strategy

Implementation
There is no new money.  However, accurate diagnosis, proactive management, review of home oxygen therapy, and provision of pulmonary rehabilitation in line with the strategy should bring about savings and deliver better quality services.

Strategic Health Authority respiratory clinical leads have a remit to:
  • Establish a steering group including primary, secondary and intermediate care clinicians and managers to oversee service development
  • Work with PCTs to help the development of communities of practice at a local level
  • To support improvement projects to gather further information on innovation and gaps in the evidence
  • Report annually on progress
Metrics
  • Quality of diagnostic services: e.g. adequacy of diagnostic spirometry
  • Data recording: e.g. recording of MRC dyspnoea score and multi-component indices
  • Structured care: e.g. frequency of reviews, provision of smoking cessation advice, pulmonary rehabilitation, personalised care plans including self management
  • In patient care: e.g. admission rates and duration, proportion managed by a chest specialist, readmission rates and mortality within 3 months post admission
  • Accessible, high quality local services: e.g. availability and outcomes of the oxygen assessment service
A phased approach to implementation of the recommendations

Work with the Lung Improvement Programme to:
  1. Ensure that existing people with COPD have a correct diagnosis, and are managed according to the most cost-effective and evidence-based interventions, including smoking cessation support, regular review, use of care plans, reducing hospital admissions and length of stay, and reviewing home oxygen registers
  2. Identify people with milder stage of COPD and develop prevention strategies to stop disease progression
  3. Increase the proportion of integrated care organised in the community
