Self management  and self care

Recommendation 11: Good-quality information should be provided at diagnosis and delivered in a format that any person can understand.
Recommendation 16: People with COPD should be encouraged to learn how to help manage their condition themselves and how to have positive interactions with healthcare professionals and others about their condition. They should also be encouraged to engage with others who have COPD in order to promote exchanges of information, support and advice.

Information underpins self-care
Tailored to the individuals’ disease severity and personal goals, information should include advice on quitting smoking, remaining active, using medication, and self-management of exacerbations. Useful sources of information include the British Lung Foundation, Patient UK, NHS Direct, NHS Choices, European Lung Foundation. Information Prescriptions enable patients to collate information specifically tailored to their needs.

Personalised care plans provide a record of self-care plans including clinical information and planning, personal goals and social needs.

  COPD Action Plans summarise self-management advice on recognising deterioration, and the appropriate to take. Examples are available from British Lung Foundation, Electronic COPD Action Plan software, Healthy outlook from the Met Office.

Support for self-care. Healthcare systems need to ensure flexible access to professional support. The Expert Patient Programme empowers patients to work in partnership with their clinician. Lay support groups (e.g. Breathe Easy) are highly valued by people with COPD.
