Are patients with Chronic Obstructive Pulmonary Disease at Pendle Medical Partnership managed in accordance with recommended national guidelines? (ID 284)

Abstract

Aim: This audit aims to improve the uptake of the Pneumococcal Polysaccharide Vaccine (PPV), annual influenza vaccination and monitoring in the management of Chronic Obstructive Pulmonary Disease (COPD) at Pendle Medical Partnership (PMP), against National Institute of Health and Clinical Excellence (NICE) Guideline 115: Chronic obstructive pulmonary disease in over 16s: diagnosis and management.

Background: COPD is the second most common lung disease in the UK comprising of 4.5% of the over 40 population and plays a significant financial burden on the NHS at £1.9 billion in 2016 (Burki, 2017; NHS Digital, 2017). Although COPD cannot be cured or reversed, the effect of the disease on daily activities can be reduced through optimising medication and drastic lifestyle changes (Gershon et al., 2016).

Method: This is a retrospective primary care audit carried out using data between March 2019 to April 2021 with COPD. The criteria were selected from NICE guidelines and the target standards were adapted from national standards.

Results: This audit includes results from 426 patients (211 females and 215 males) with COPD. PMP achieved the target standard for offering COPD patients the PPV and annual influenza vaccine. 94.6% of COPD patients were offered the PPV, while 95.3% and 91.5% of patients were offered the annual influenza vaccine between 2019-20 and 2020-21 respectively. However, only 72.8% and 41.8% of patients had their annual review between 2019-20 and 2020-21 respectively, which was lower than the target standard.

Conclusion: The management of COPD patients at PMP is generally in line with NICE guidelines in terms of the specific criteria tested, however, improvements can be made to conduct reviews during the COVID-19 restrictions through implementation of virtual reviews which will further optimise the management of these patients.
