The PCRS blueprint for Neighbourhood Health in COPD outlines how Neighbourhood Health models could organise COPD care around integrated local services. It proposes clear points of contact, population health management, coordinated, collaborative and personalised care planning and access to diagnostics, community respiratory teams and multidisciplinary support. It advocates for holistic, patient-centred care, as well as appropriate funding, diagnostic capacity and strong collaboration between health, social care and community partners.

The main blueprint is supported by a number of supplementary documents which together provide local healthcare commissioners, managers and services with what they need to plan and implement effective Neighbourhood Health models for COPD. Supplementary documents include:

• A narrative document which provides more information and guidance on interpretation and use of the blueprint,
• An example blueprint which depicts how someone with COPD could potentially flow through the proposed model, and
• A whole system blueprint for effective Neighbourhood Health implementation from local delivery to national strategy.

Please note: PCRS currently has nine Primary Care Networks (PCNs) piloting this model in their local area. Outcomes from this pilot will be made available via the PCRS webpages in January 2027.
