Primary Care Sheffield
Abstract
Referrals from primary to secondary care are a daily occurrence. Both primary and secondary care have an immense workload. At times it can feel like there is a gulf between the two. This service was developed to help build joined up care, building a bridge between primary and secondary care.
Routine secondary care referrals are made via the NHS e referrals service(eRS). In Sheffield Primary care providers are encouraged to submit these via the CASES teams in various specialities, including Respiratory Medicine. Referrals are peer reviewed by a General practitioner who has received mentoring and support from Specialist consultants at Sheffield Teaching Hospitals NHS Foundation Trust. The referral is either forwarded to the hospital or returned to the GP with advice.
CASES is an elective care redesign programme, aiming to reduce the pressure on elective services and ensure that patients receive their care in the most appropriate place from the most appropriate clinician. This service has been running for several years, via Primary Care Sheffield(PCS), a ‘social purpose’ company set up by primary care in Sheffield to provide support in delivering world class care. With the upcoming changes in primary care and the focus on advice and guidance services this service is a key part of primary care in Sheffield. We feel that other providers might be interested to know about this service.
