Abstract

Interest in remote primary care has escalated by necessity during the COVID-19 pandemic. We report on uptake and use of a text message based questionnaire system used to gather assessment data supporting patients with COPD. It provides both valuable clinical data and a pathway for remote patient engagement. The COPD ‘Florey’ questionnaire service is sponsored by GSK and was co-developed with accuRx.
The accuRx system is familiar to HCPs in primary care, being utilised in 95% of practices across England, Wales & NI – with the greatest usage in NHS England. The questionnaire includes 15 questions which includes specific assessment of CAT, MRC, smoking and exacerbations. Patient response data can be integrated into patients’ records and there is functionality for batch send to a full patient register and structured reminders.
Usage rates have consistently risen since launch in October 2020. Up to June 2021, questionnaires have been sent to 75,000 unique patients and the average patient response rate is 61%. The COPD service has been used by 31% of all practices using the accuRx platform. In March 2021, 8% of all accuRx Floreys sent within the platform were for COPD.
Responses have been used to identify high priority patients and initially stratify practice population based on symptoms and clinical need. It also provides some dynamism in the HCP/patient relationship, a pathway for simple longitudinal follow-up of key COPD clinical markers and a driver for appropriate intervention in support of the regular COPD clinical review cycle.
This work highlights the value of an integrated remote care pathway for prioritisation and engagement of patients. It offers a potentially efficient pathway to explore innovative approaches to patient support, not just during the pandemic but also in future routine care.
